How to Apply for Your Rebate

To apply, send pictures or copies of:

  • Completed Appliance or Weatherization Rebate Form
  • Detailed receipt or contractor invoice
  • Energy Star® label for appliances
  • Product serial number
  • Your name, phone number and address
